18. Manische Episode und bipolare affektive Störung

Zusammenfassung

Die bipolare Störung ist charakterisiert durch den Wechsel zwischen den Polen Manie und Depression bzw. zwischen manischen und depressiven Episoden oder Zyklen. Bei der Behandlung einer akuten manischen Episode spielt die Pharmakotherapie eine zentrale Rolle; sie dient der Stabilisierung der Stimmung, des Antriebs, der Schlafregulierung und soll dem Auftreten weiterer Phasen vorbeugen (Phasenprophylaxe). Zum Einsatz kommen Antipsychotika der zweiten/dritten Generation und stimmungsstabilisierende Antiepileptika. Für die Behandlung der akuten Manie kann Aripiprazol aufgrund seines Nutzen-Risiko-Profils als Medikation der ersten Wahl gelten; Olanzapin, Quetiapin und Risperidon sind gleichrangig als Wirkstoffe der zweiten Wahl anzusehen. Bereits ab der zweiten Episode soll zur Rückfallprävention phasenprophylaktisch behandelt werden. Auch hier kommt Aripiprazol als Mittel der ersten Wahl zum Einsatz; Lithiumsalze sind Mittel der zweiten Wahl, gefolgt von Valproinsäure. Bei nicht ausreichendem Ansprechen auf eine Monotherapie kann die Behandlung mit mehreren Wirkstoffen notwendig werden.
