Mit zunehmendem Alter erhöht sich die Inzidenz einer symptomatischen Bradyarrhythmie und häufig ist die Implantation eines Schrittmachers notwendig. Die akute medikamentöse Behandlung (oder das Absetzen potenziell ursächlicher Medikamente) ist zwar für eine kurzfristige Stabilisierung des Patienten in der Akutsituation geeignet, verhindert letztendlich aber selten den Bedarf eines Schrittmachers. Das zeitgemäße Management besteht heute darin, eine temporäre Schrittmachertherapie zu vermeiden (mit Ausnahme besonderer Situationen wie Myokardinfarkt und perkutaner Aortenklappenersatz) und den erforderlichen Schrittmacher idealerweise im Rahmen eines einzigen Eingriffs auch außerhalb der normalen Behandlungszeiten zu implantieren. Unabdingbar ist es, eine eindeutige primäre Diagnose zu stellen, um ein individuelles personalisiertes Vorgehen bezüglich der optimalen Devicewahl (Schrittmacher, ICD, CRT) und -programmierung sowie der langfristigen Planung von Aggregatwechseln zu gewährleisten.
